Passport Application Procedures

  • New passport application forms were introduced on 1 July 2008. They can be identified by the letters F or G located below the small barcode at the top of each page. Old forms will not be accepted after 30 September 2008.
  • You will be required to present additional documents if your name shown on your Australian passport is different to your name shown on your Australian citizenship certificate or your name registered with an Australian Registry of Births, Deaths and Marriages.
  • If your passport expired more than 12 months ago you will be required to provide original documents as proof of your Australian citizenship plus original documents which show your place of birth and gender.
  • You may be eligible for a streamlined application form if your passport is due to expire within the next 12 months or if it expired less than 12 months ago.
